Her appt was for the following week. This is Ashley's scenario. Although I was dropped from the case, I learned my lesson. ALL PT CONTACT MATERIAL IS PLACE IN THE CHART AND AN MD SIGHNS IT. I made the jump to an EMR 2yrs ago so this is easier, but when I started my own practice 3 years ago; the staff wrote everything down and placed it on the chart and I signed it. The American Medical Association is no longer designating CME hours for AMA Category II CME credit. However, physicians themselves may self designate learning activities as Category II CME credit hours if they feel it is of sufficient educational merit and meets the formal definitions of continuing medical education.
