Did one like that years ago. She was about 20 weeks, and her cerclage had failed for much the same reason. She was a DES patient.

We did a transverse incision (a large one) and at one point had to exteriorize the uterus. Things ultimately went okay, and she delivered by c/s near term - I think a little premature because of some preterm labor, but no problem with the baby.

I think it just depends on what everyone consents to, with much "informed" before the the consent.
