Re: What is the diagnosis?

LP showed no bacteria, no white cells 3 red cells (I haven't done an LP for 5 years, so I needed two attempts, my SMO (Resident) found several excuses not to do it herself :-)-O) Protein was elevated, glucose low. Indian Ink negative. Direct Microscopy to come today.

HIV was reactive and has been sent for confirmation.

I feel this is Tuberculous Meningitis or HIV encephalitis.

I have put her on full TB Rx and of course we added Potassium to her drip :-)-O, but this I don't thinks has much to do with it. I have also put he ron some steroids.

The nearest CT is 750 km away by road transport (remember, I work in rural Africa :-)-O). If she doesn't improve I might send her for imaging after all.

But, what do we do about the pregnancy?

I'll keep you informed.

greetings, el

The American Medical Association is no longer designating CME hours for AMA Category II CME credit. However, physicians themselves may self designate learning activities as Category II CME credit hours if they feel it is of sufficient educational merit and meets the formal definitions of continuing medical education. OBGYN.net believes these interaction in this forum meets these criteria. For further information see the AMA web site.
