Thank you for your comments. Your opinions are most helpful, since I do not know the answer.

I did not do the first cerclage. The operative report describes the membranes to be visible on speculum examination, protruding slightly past the external cervical os, with the cervix open 2 cm. I think that qualifies for a "rescue cerclage". A foley wasn't necessary. When the cervix was grasped, it was pulled over the membranes. My associate did the cerclage while I was at a genetics meeting, last week.
