I'm talking the patient who is definitely eu-estrogenic with ERT but who has gone on to research extensively by talking to friends, etc. as well as scouring the internet for alternatives to estrogen. She tried the natural progesterone cream pushed by John Lee M.D. and his minions but - surprise! - this by itself wasn't the perfect solution. She (age 42) complains of difficulty concentrating, memory problems, as well as libido and other problems. Yes, I have tried Estratest in both dosages. Her FSH level a couple of years ago (she is status post TVH) was >100. My concern is she's now about to spend unknown $$ on salivary steroid hormone assays available through the web site below, recently given airtime on - of course - Oprah! http://www.salivatest.com/store/index.html <http://www.salivatest.com/store/index.html> My problem is that this has not been through the prospective/randomized/double blind/placebo controlled gold standard evaluation. What do you folks do with these patients? Does anyone actually titrate ERT or HRT with biochemical assays instead of by improvement in the patient's symptomatology? I'm trying to be open and told the patient I'm certainly willing to try the compounded estrones/estradiols/micronized progesterones/etc. Suggestions? The American Medical Association is no longer designating CME hours for AMA Category II CME credit. However, physicians themselves may self designate learning activities as Category II CME credit hours if they feel it is of sufficient educational merit and meets the formal definitions of continuing medical education.
