Well - this goes to the heart of what second stage really mean - I tell my patients not to push until they feel the urge to do so -that where start the clock - yeah I know that with an epidural -that's not the same story but nevertheless I tell them not to push until the presenting part is way down - lots of pit - will do it for you - BTW - I haven't seen the editorial about forceps -not yet on line -

The American Medical Association is no longer designating CME hours for AMA Category II CME credit. However, physicians themselves may self designate learning activities as Category II CME credit hours if they feel it is of sufficient educational merit and meets the formal definitions of continuing medical education.
