There is not a shadow of doubt that multiple gestation increases obstetric risk.

ACOG definition of Level I facility would require the ability to do a C/S within 30 minutes from the decision to do so.

Ergo, a delivery at the "hamlet" is quite out of the question.

A G3P2 wishing to deliver twins vaginally wihout any other risks should be allowed to do so in a level I facility, ie at your hospital.

Should she want a section I'd go along with it too.
