In difficult cases I've give indigo carmine intra-operatively and view the ureters directly (open or with scope), and will often get a post-op IVP in the recovery room. If I think bladder laceration, I'll use sterile milk.

I've never had anyone tell me I can't do cystoscopy (I'll use the mini-hysteroscopy set) but if the patient is supine it gets to be a PITA to reposition, inject indigo carmine and wait for the flow through the ureters. If the patient is in stirrups I'm usually more predisposed to cystoscopy. All Burch, MMK and paravaginal repair do get cystoscopy, though, regardless.

Yes, I bill for the cystoscopy.

do you cystoscope any/all pelvic surgery cases or just your "difficult" ones where there is doubt regarding the integrity of the ureters? what about enterocele repair/support procedures involving plication of the uterosacrals? does your hospital require separate crudentials for cystoscopy or include them under the general "endoscopy" heading. do you bill for the scope or just assume it is covered in the global description of the procedure?

just curious what the rest of the list is doing. no gyn at our hospital routinely scopes but doing just that has been stressed at the last three CME meetings i have attended. i started scoping routinely last week and it has raised some eyebrows,caused grumbling that it ties up cystoscopy equipment. what do you think? thanks
