Not that I can see on CXR. No sputum to check, either, for culture. Besides, she had been on Augmentin recently. I'm treating her as an outpatient right now, as she is afebrile, functional and doesn't want admission at this point. She'll follow up with me Monday, or sooner if need be.

Obliterated right diaphragm, air bronchograms, loss of cardiac shadow, just in the lower right base. Afebrile, minimal sputum. I trialed her on Augmentin and it failed. ER doc recommended Omnicef 300mg BID for 10 day, and it is category B in the PDR.

What does the CXR look like? Consider atypicals--try e-mycin or Azithromycin. Note: Epocrates ID recommends Amox/CL plus e-mycin as 1st choice in CAP, Azithromycin as second choice. Clarithromycin is third choice but it is pregnancy Category C. Also, don't know about your neck of the woods, but we've been getting some nasty viruses up here in the Midwest that hang on forever.
