If request for C/S is an acceptable medical indication to ACOG, then it should come as no surprise that epidural by request is the same. The ethical issue of witholding pain relief is significant. One could use narcotics but note the opinion specifically says epidual. The analogy for me is to say, "I don't want to take NSAID's for my pelvic pain so don't deny me surgery". My crystal ball says that hysterectomy by request is next. News at 11.
