Update:

I decided to use 200 mcg vaginally. Patient received a single dose at 5:30 p.m. and started contracting about 3 hrs. later. She delivered at 3:00 a.m.

The umbilical cord was twisted several times at the abdominal insertion, and the entire cord was extremely edemitous--about 4-5 cms.
