Interesting case that would like some suggestions. Our midwife saw an 89 year old woman yesterday who complains of overly heightened libido during the past few years. She complains of an almost heightened awareness to the anterior vulva, and clitoris that borders on painful. The discomfort radiates to the sacrum. She has not been sexually active since her husband's chronic illness began about 3 years ago. The patient is otherwise healthy and on no meds. She has seen a variety of gynecologists who have biopsied (without definitive findings0, performed a hysterectomy without oophorectomy, used a variety of topicals, and suggested that she masturbate regularly. None of these have been helpful. On exam the area appears prominent for being post-menopausal but not virilized. We have drawn serum testosterone but am at a loss of how to proceed. Any suggestions?
