Canadian groups release guideline on breast cancer chemoprevention

WESTPORT, CT (Reuters Health) - Two Canadian health groups have released a new guideline that should assist women and their physicians in deciding whether tamoxifen and raloxifene treatment is warranted in reducing the risk of breast cancer.

Overall, the guideline recommends that for women with a Gail risk assessment index of <1.66% at 5 years, tamoxifen preventive therapy is not recommended. For women with a Gail risk assessment index of 1.66% or greater at 5 years, the pros and cons of tamoxifen preventive therapy need to be discussed.

The guideline was produced after a review of relevant literature published between 1966 and 2000, and represents a joint effort by the Canadian Task Force on Preventive Health Care and the Canadian Steering Committee on Clinical Practice Guidelines for the Care and Treatment of Breast Cancer.

The groups also addressed two other issues. Based on the current evidence, they conclude that raloxifene probably should not be used for breast cancer prevention outside of the clinical trial setting. While the Gail risk assessment index can be useful in determining whether chemoprevention is warranted, they do not recommend this tool for use by physicians for screening or case finding.

Dr. Mark Levine from McMaster University Health Sciences Centre, Hamilton, Ontario, chair of the Steering Committee, and colleagues prepared the guidelines, which appear in the June 12th issue of the Canadian Medical Association Journal. The current guideline is part of a series of breast cancer guidelines produced by the groups.
