Surgical--Hyst, with BSO (she's 52!) offered/strongly recommended. The method, TAH, or LAVH, or LSH (lapscope supracervical) depends on individual assessment of the patient and your preference.

Medical--if she is premenopausal (I assume that she is), you should be able to control her with DepoProvera, or Lupron, or the pill. She likely needs to be rendered amenorrheic while replacing iron, and once her Hgb. is corrected, controlled with one of the above medical methods, or surgery.

All in all, my gut says to stop her bleeding short term, replace iron, and then do the hyst.
