The premise embedded is that the totality of encounter is captured in the clinical note. That's a nfw. The clinical note is true to the extent of actuality that was considered worth remembering. Or, of after-thoughts that would have been nice, should it have happened. Or, what might happen at next encounter. Or, added value delivered at encounter that algorithm did not include, but were worthy of chargable compensation.

Prediction: there will be continuing minor disagreements in your world. And, mine.

The American Medical Association is no longer designating CME hours for AMA Category II CME credit. However, physicians themselves may self designate learning activities as Category II CME credit hours if they feel it is of sufficient educational merit and meets the formal definitions of continuing medical education.
