I have a 51 yo patient who has been on Evista for the past year. Menopause in mid forties, previously on HRT, but switched because of breakthrough bleeding. She was diagnosed with lupus about 3 months ago, and her rheumatologist discontinued the Evista, saying that he felt it increased her risks, due to the SLE, but then told her, "talk to your gynecologist about whether or not you should go back on it." She has no evidence at this time of vasculitis or renal problems. I'm aware of the suspected relationship with oral contraceptives with increased of risk of thombotic incidents in lupus patients, and of course the problems in pregnancy, but have not been able to find similar concerns regarding HRT or SERMS. Anyone have any pearls? She had a normal DEXA scan 1-2 years ago, is a non-smoker and a little on the chubby side, therefore not at a greatly increased risk of osteoporosis. Oh, and of course, she is a nurse and wants as much specific information as possible.

The American Medical Association is no longer designating CME hours for AMA Category II CME credit. However, physicians themselves may self designate learning activities as Category II CME credit hours if they feel it is of sufficient educational merit and meets the formal definitions of continuing medical education.
