I performed a laparoscopy today on 37-year-old G0 with chronic pelvic pain and a history of endometriosis, and did chromopertubation. Using the Kahn’s canula, there was immediate uptake of Methylene Blue into the myometrium. Thinking the end of the canula had somehow invaded into the myometrium, the Kahn’s canula was replaced by a HUMI intrauterine manipulator, and again I injected Methylene Blue. The uterus got bluer, until finally there was evidence of flow through the tube. Does this rapid uptake of the dye suggest adenomyosis? Aside from a benign looking 3-centimeter simple cyst I could not find evidence for this patients chronic pain. The most I could see was a ‘tight’ looking peritoneum under her left ovary, where some of the pain complaint was, but no gross endometriosis, and it felt smooth with the probe. Thanks Rick
