was a letter - acknowledging the earlier study as the basis of their own investigation in their own neck of the woods. first study was in UK, unless Malcolm says otherwise. it is of interest to compare attitudes around the world.

can't speak for editors of the Lancet. just my opinion and as always, i could be wrong.
