89 YO woman referred from a new, just out of training urologist due to urinary retention. She has had multiple procedures--a couple of "bladder tacks", TAH, etc--all per history, no records. Anyway, she just moved her, and had performed intermittenet self cath BID for 4 years, with about 500 cc residual each time. The urologist noted that she had a large cystocele/rectocele and wondered if a pessary might help. Her exam is pretty much that--maybe an enterocele, but both are within the introitus. I am not really aware that a pessary to reduce the cystocele/rectocele will help in this situation. I don't belive this is a partially obstructed urethra from the cystocele; in fact, she looks a bit worse than average for someone at this age, and whose had a bunch of children. PS she's on estrogen. The American Medical Association is no longer designating CME hours for AMA Category II CME credit. However, physicians themselves may self designate learning activities as Category II CME credit hours if they feel it is of sufficient educational merit and meets the formal definitions of continuing medical education.
