This is a tough case, Caitlin.

Her endometrium needs evaluation, and her abnormal Ca 125 needs explanation, though it probably isn't anything bad. What she needs is a TAH BSO, but obviously that is not what she wants.

Consider:

1. Spelling the above out, nicely, firmly, and with a nice dictation/note in the chart. 2. What about continuous OCPs or DepoProvera or Depolupron to stop her bleeding?

If she does not comply reasonably, or if you are uncomfortable, tell her so, and if you feel the need to not care for her, dismiss her in writing.
