I believe you may have discovered the reason Robin's Medline search failed. I just did a very quick search on OBGYN.net's Knowledgefinder free Medline search engine and found several references. I'm sure many more could be found by expanding out search through individual articles. I used "squatting in second stage labor alternative birth positions" as my search criteria. There is quite a bit of information on preserving the perineum in the nurse-midwifery literature, which is included on Medline.
