Thank you all for your responses. To answer some of the questions and comments you made (for some reason the "respond to this message button was missing from the posts yesterday) Geff: I too think this is a genetically abnormal baby with severe IUGR and I am treating her accordingly. SHe is now 28 weeks and I've begun antenatal testing. Charlie: I too thought "horse" when I heard the hoofbeats and not "zebra" Joe: The patient and her husband have refused genetic studies, even when I said "you may be carrying an abnormal baby that cannot live under any circumstances -- and we may do a c-section in labor and you will still have a baby that will die." Art: the MFM said he didn't do dopplers (remember, he's saying she was only 23 weeks when he scanned her last week) because it was "too early" , but "everything looks good, the baby is symmetric, the placenta looks good and the fluid is normal". (BTW, when I scanned her, I saw a lot of echogenicity in the placenta and called it a grade 3, and the AFI was a hair low) Efrain: My RDMS certified tech did the first scan -- and I took a look myself because the patient and her husband were so anxious. I measured the fetal pole again, getting a number very consistent with my techs 3 measurements -- and all 4 of us in the room could see the heartbeat. I scanned the patient myself at 8 weeks, too. As to why not get a new MFM: There are only 2 MFMs (practicing together) in the nearest small city, and the second nearest city is 2 hours away, over a mountain pass that has been intermitently closed all week (the 2 inches of snow pedicted to fall yesterday looks an awful lot like a foot in my driveway!). The American Medical Association is no longer designating CME hours for AMA Category II CME credit.
