there is no lab test with a predictive value of 100% - that is you still need to utilize clinical jusdgement - somewhat akin to ben kenobi's admonition - "trust your instincts."

with regard to sonar - it is the absence of an intrauterine pregnacy which is still THE significant finding which should point the way to the diagnosis.
