The CIGNA medical director asked me to transfer her. I told him that my partners and I did not feel that it was appropriate given her multiparity and advanced dilatation, and only 32 weeks by poor dates.

He said OK.

While it was not explicitly stated, I can think of no other reason to request a transfer to an "in-network hospital" (his words) other than economics. She is at a hospital equipped for a 32 weeker.

Robert, my gut says we could move her by ambulance easily, with little risk of delivery. I would only want to move her if she wanted to be closer to home, as she lives 45 minutes away. OTOH, she's not been too responsible about this pregancy, either, and she presented with vague side pain and was 5 cm.

There is no reason that I should transfer her from my perspective unless she wishes to do so, and then I would be reluctant because of the possibility, however remote, that she would rapidly labor/deliver en route. Why take a chance?

The American Medical Association is no longer designating CME hours for AMA Category II CME credit.
