I also had a couple of women with this problem over the last several years. I was told by one of the original investigators in ablation procedures that it actually takes several months for the maximal scarring to occur in the endometrium, and that this does not necessarily mean it is a failure. However, I did find from my experience (non statistical) that these women seemed to be more likely to be one of the failures when they resumed their menses soon after the procedure like this.
