Well - this is the type of case that really deserves a tumor board discussion. Having said that - I will go out on a few limbs! I am open to any and all comments and criticisms!

Ou guideline for probability of lymphatic involvement is usually based on how far through the myometrium it went. So ... if the myometrium was 100% not involved, then even though you morcellated it, you may not need any adjuvant therapy. By the time you were morcellating, you had already clamped, cut and ligated the primary venous and lymphatic drainage. So, one can argue that nothing needs to be done.

Having said that - It can also be argued that you should do an abdominal (laparoscopic if you are really good at that) and do BSO and pelvic/obturator lymph nodes. If all negative - then you are done.

Anyone who would argue for something like Megace or Progesterone therapy?

The American Medical Association is no longer designating CME hours for AMA Category II CME credit. However, physicians themselves may self designate learning activities as Category II CME credit hours if they feel it is of sufficient educational merit and meets the formal definitions of continuing medical education. OBGYN.net believes these interaction in this forum meets these criteria. For further information see the AMA web site.
