Ob care, previous Cesarean, failed VBAC, Cesarean delivery, but is anyone getting paid extra for the new CPT code for failed VBAC's?

Today, a multip prev LTC/S planning a VBAC, having had 2 successful VBACs in my practice already, presented in early labor at 3 cm.  Upon AROM by my partner, which I, too, would have done, a hand and forearm came right through the cervix.

She had a repeat C/S (this isn't posted to discuss that--he called me and told me that she needed a section, and I have learned that even if I don't agree, and I did, that once the doc on the spot makes a call, it is bad news to reverse it).

Would you bill her for:

Ob care, Cesarean

or

Ob care, previous Cesarean, failed VBAC, Cesarean delivery

The American Medical Association is no longer designating CME hours for AMA Category II CME credit.
