Well, when I dictated the delivery summary, I just dictated the indication for the forceps operation, how it went, and the repair--with no mention of the Mom.

The next day, after a heart to heart with the patient (and a RN witness, no Mom), I wrote what we discussed:

1. Reason for forceps 2. Nature of the tear and prognosis 3. Baby outcome (mild facial palsy on one side, which the ped thinks will go away. One 3 mm abrasion on the opposite side, and no marks, suprisingly) 4. Mom's dissatisfaction, and her request that I not see her daughter. Specifically, I asked and received permission to continue to see the daughter and asked and received permission to do the requested circumcision.

The others have responded right on the mark about the multi-racial baby. This was not meant to offend, but to give the relevant background. Notice how many of the US listers immediately understood my situation as if they could read my mind!
