G5P4, all previous SVD. Labour slower this time, has urge to push after 4 hours of active labour, pushes with no result x 3 contx. VE to assess= hand and forearm in front of vertex -1 to spines. This happened recently. I tried to pinch the fingers to get the baby to pull the hand back, no room to push the arm and hand back in dorsal or lateral positions. Moved to hands and knees, gentle pressure on the vertex provided enough room to move the hand and arm back. Vertex slid back onto cervix with digital control, 8 cm. Within 5 minutes the baby was born. Slight bruise below antecubital fossa, good nerve and muscle function of arm. Baby delivered with hand on cheek. No analgesia during labour and birth. The American Medical Association is no longer designating CME hours for AMA Category II CME credit. However, physicians themselves may self designate learning activities as Category II CME credit hours if they feel it is of sufficient educational merit and meets the formal definitions of continuing medical education.
