If any coding people or insurance company people (Jay Kulkin) have an opinion on how to submit this claim--i.e. electronically or manually, with or without a letter and/or op note--it would be appreciated.

I think we all agree that the correct coding is subtotal hyst (was it 58180) with a -22 modifier at whatever price you want.

Clearly, following the CPT rules, if you do more work, document it, code it correctly, you should charge more and be paid more. Again, to any coding/insurance people, what's the skinny (Jay, Bernard Greenberg if you are out there?)?

The American Medical Association is no longer designating CME hours for AMA Category II CME credit. However, physicians themselves may self designate learning activities as Category II CME credit hours if they feel it is of sufficient educational merit and meets the formal definitions of continuing medical education.
