I once had a "resistant" trich infection - she has been treated over many months (probably 1-2 years) by other physicians, with persitant recurrence.

After confirming presence of trich on the wet mount, I gave her a course of Flagyl for 10 days, and saw her again at the conclusion of treatment. At that time she was asymptomatic but I again saw the Trich on the wet mount, although in much lower numbers than before. I then placed her on Metrogel for a month, and that seemed to have "cured" her. She did not have any recurrence afterwards.

Thus I think this "recurence" is most likely incomplete treatment, for whatever reasons. Solution is to persist until eradication. However, I am not sure how I would handle this in a pregnant patient.
