Kevin:

58150-22 is the appropriate code to use; set whatever price you would like (since the carrier will do what it wants anyway). I hope that your op note has detail in it to reflect the justification for the "-22" modifier. As a matter of course, when I have a tough case in which I want to bill more, I say it in the op note.

I don't think that billing another procedure, even with the -59, is going to fly.
