Dear listmembers, I would appreciate any suggestion on the following case of cervical agenesis. 17 YO patient with primary amenorrhea and recurrent pelvic pain. Physical exam: normal vagina with a "normal" vaginal vault (cervix not visible). Body of the uterus and ovaries are normal. Transvaginal sonography: Body of the uterus and ovaries are normal. A not well-defined area between the uterine isthmus and the vaginal vault, compatible with a cervical stump. MRI: Body of the uterus and ovaries are normal. A thick and homogeneous area (15 mm height and 12 mm anterior-posterior) between the uterine isthmus and the vaginal vault, compatible with an hypoplasic cervical stump with no cervical canal. Laparoscopy: Body of the uterus and the ovaries are normal. Scanty amount of blood in the pouch of Douglas and multiple nodules of endometriosis of the pelvic peritoneum. What kind of surgery would you perform on this patient? Mario
