Yes, I would say it is long, keeping in mind that it is a first time vaginal birth and includes emotional counseling to begin effective pushing. For a good portion of that time she "thought" she was pushing hard enough. Progress determines our actions more than time does.

I don't know about it being an explanation for the rate, but long second stage obviously contributes. The client we have been discussing had her first c/s without labor at all.

Of course we do. Most labors I attend are pretty straight forward. VBAC birthers are the higher risk woman among our low risk clientele.
