After 25 years of doing ob, of course I've seen babies expired with true knots, prior to presenting in labor. However, I've not "suddenly lost one" in labor that I've been monitoring due to a true knot. Like most others, I've delivered many a baby with a true knot with nary a problem in labor, except occaisional variable decels that aren't usually worrisome.
