Appropriate off-label use is generally based on sound, scientific evidence. Scientific information on misoprostol addresses off-label use for cervical ripening and induction of labor in pregnant women who have unfavorable cervices and as an agent in termination of early pregnancy. These indications are discussed in ACOG's Committee Opinion #228, Induction of Labor with Misoprostol (November 1999), and ACOG'S Practice Bulletin #10, Induction of Labor (November 1999). Misoprostol use is not recommended for cervical ripening or inducing labor in women who have had prior cesarean delivery or major uterine surgery because of the possibility of uterine rupture. Women treated with misoprostol for cervical ripening or induction of labor-should receive fetal heart rate and uterine activity monitoring in a hospital setting until further studies evaluate and confirm the safety of outpatient therapy. ACOG's Committee on Obstetric Practice and Committee on Practice Bulletins are further deliberating this issue at their October meetings. Fellows will be informed of any necessary action or recommendations that may result from these discussions. The American Medical Association is no longer designating CME hours for AMA Category II CME credit. However, physicians themselves may self designate learning activities as Category II CME credit hours if they feel it is of sufficient educational merit and meets the formal definitions of continuing medical education.
