I was always a proponent of AROM ASAP - before Pit if everything was favorable. then I had a G4P3 woman at 3-4 cm - and the head *seemed* to be against the cervix... So ... AROM and in under 10 minutes there was this deceleration - and yes, indeed, I had a cord prolapse. As I was sectioning this multip because I did the AROM - I got a bit scared off. So I went back to - more often than not - waiting to AROM until there were contractions & I could do it during a contraction. So - if real certain of the stability of the station - then AROM, wait 1-2 hours and start Pit if needed, or start Pit, AROM as soon as contractions begin - and back off on the Pit in most cases...
