Option # 1: Stop the pitocin, feed her, use cervical ripening agent (cervidil or cytotec), monitor all night.

Option # 2: BPP--if reassuring send her home with kick count sheet

Option # 3: Stop IOL, feed her, and reassess in a.m.--gives patient and family time to consider options

Option # 4: The OB I work with would be willing to offer patient C/S for failed IOL especially if EFW>4000 gms.

I personally would have tried to talk the patient of of an IOL with an unfavorable cervix. However we often have patients who are insistant and its hard to say "no" when they show up in L & D with their bad packed and family there.

The American Medical Association is no longer designating CME hours for AMA Category II CME credit. However, physicians themselves may self designate learning activities as Category II CME credit hours if they feel it is of sufficient educational merit and meets the formal definitions of continuing medical education. OBGYN.net believes these interaction in this forum meets these criteria. For further information see the AMA web site.
