I use prophylaxis only for C/S in labor or with ROM, not for scheduled C/S (Ancef, 1gm). The CDC guidelines silence on the subject of benefit or lack of for GBS screening of scheduled C/S resulted in my not doing GBS cultures on this group of patients. My rationale was why do a test that is not going to benefit the patient and cause more anxiety. Unfortunately, the pediatricians at my hospital want to know GBS status on all infants. Challenging them to give me evidence of the benefit resulted in no evidence. However, their response is that if there are no cultures, they will treat the infant as GBS unknown, doing the required sticks for serial CBCs, which I feel is unwarranted on these infants. As a result, I am starting to do cultures now and because of the inevitable positive, these patients will be brought in early and receive their PCN/Amp/Clindamycin at least 4 hours before I cut.
