Our practice has just taken into care a 30 year old woman who wants to have an amniocentesis done. She is fully cognizant of the risks of miscarriage vs. malformation, but states that it is absolutely unacceptable to her and her husband to have a baby with a genetic defect. Our genetics clinic and OB's up here will *not* do an elective amnio on a woman unless she is aged 35 or more. Otherwise the route is maternal serum screen and amnio prn. Part of the rationale is, no doubt, use of resources and cost-effectiveness, but the genetics clinic also expressed ethical concerns (i.e. people wishing amnio to determine gender and termininating if wrong gender). This woman is a US citizen and, therefore, wishes to go to the US to pay for an amnio. The closest major centres for her from here are Detroit and Buffalo. Without getting into a large ethical discussion about this whole issue, I would like to know a) if it's possible for her to self-refer to an obstetrician in the US for amnio and b) names of recommended obstetricians in either of these two centres. Although the woman desparately wants this amnio, and will travel and pay to have it done, she also wishes to have a competent practitioner. The American Medical Association is no longer designating CME hours for AMA Category II CME credit. However, physicians themselves may self designate learning activities as Category II CME credit hours if they feel it is of sufficient educational merit and meets the formal definitions of continuing medical education.
