Re: pregnancy and fibroids

The earliest complication of fibroid during pregnancy in my practice was at about 16 weeks I believe. It was the worst antepartum one I have had. She ended up on a PCA pump of Demerol with a continuous rate for low level of narcotic then the PCA bolus as well. After about 10 days her narcotic requirements dropped dramatically. The PCA was removed, she did well on tylenol with codeine, was sent home that day and was off the codeine in under 48 hours. Basically as soon as the active degeneration was complete she needed nothing for pain. She went full term, needed augmentation the second stage (as I recall) and delivered vaginally. Wonderful little boy. Did a myomectomy after the pregnancy. It was still 8 cm after pregnancy involution... We had another woman infarct and infect her fibroid following a C/S around then as well. That was a tough year for fibroids & pregnancy in our practice.
