re 12 X 16 cm mass, I was taught that something of this size deserved removal, especially if discovered outside the first trimester. the risk of torsion is real and can turn an elective case to an emergent one.

re 13 yr old with dysmenorrhea/pelvic pain not responsive to OCP. I have a low threshold for Dx Scope in patients like this. Mainly because I have performed probably 4 dozen scopes on similar patients who have been passed from one doc to another or who have been ignored and found significant pathology in 80-90%. This includes endometriosis, adhesions, developmental defects, endometriosis of the appendix, etc. I appreciate your mention that you don't think the problem is supratentorial. Far too often these young ladies are treated in a condescending manner and "blown off" only to suffer for years before someone actually listens to their complaints.
