Hi, I had two interesting South Dakotans visit me today. I would appreciate your advice.

Case One

A 17 y/o G2 P1 at 26 wks with off/on right lower quadrant pelvic pain. Crampy in nature, felt like gas pains to her. A certain dull ache always was there, but got worse at times. Various GI meds didn't seem to help, so I had an US done today looking for some uterine anomaly or perhaps a GU stone. Well, the tech called me downstairs for this one. There is a 12X16cm homogenous mass arising from the right adnexa. No ascites, the liver,gallbladder, fetus, placenta, left adnexa are fine. This looks a lot like a serous cystadenoma, and likely benign given her age, and the cyst characteristics. What would you do? Surgery now or later at term? Would you need to do a C/S if the cyst is a big water balloon?

Solid or Cystic???? Sounds like a Theca Lutein cyst or a Luteoma. Does he have any evidence of masculinization?? Unless suspicious of malignancy or it is below the baby, or twists; I might wait til after delivery and spontaneous regression.

Case two A 13 y/o G0 with severe dysmenorhea. Unresponsive to last 4 or 5 cycles of OCPs and NSAIDS. Actually went to the ER last month at menses due to the pain. US then was normal. Exam today was remarkable for central uterine pain. She is not sexually active, and her mom was with her today. The does not seem to be an "above the neck" case. Any other ideas for a treatment before a Diagnostic laparoscopy? Thanks for your advice.

Is she really taking the OCP's???? Dan
