Personally, when the patient comes in for a 6 week PP exam, I do a full exam and pap as if it were an annual preventative service, even if the last pap (like when she was first seen at 8ish weeks) was only 8 months ago. In this fashion, the pap that is "due" in 4 months isn't missed. Of course, if she is bleeding too heavily, then I omit the pap, and ask them to come back.

In a former Atlanta practice in Sandy Springs with a couple of older gyns, one semi-retired (you know who), they got upset when you did a pap at 6 weeks rather than have *their* patient return in 4 months or whenever it was due. To some degree, then and is now, there is renevue to be made in having the patient return for a separate, chargeable service in 4 months.

In an ideal world, I think the 4 month visit would be cool; in practical terms, I think a 6 week pap is appropriate.
