as described, a 4.0cm unilocular ovarian cyst in a postmenopausal woman without increased pelvic fluid, debris within the cyst, irregularities in the cyst wall or solid areas is highly unlikely to be malignant. obviously it all boils down to how sure you and your patient wish to be, but we see these on an almost daily basis in our lab. many women have been followed with these now for years.
