Most trials assessing the efficacy of misoprostol for postpartum hemorrhage have small sample sizes. In general, the misoprostol group appeared to show benefit, although not reaching statistical significance (probable type II error). The fact that Patrick Ramsey feels it is not effective doesn't mean that it isn't. Large RCTs need to be performed. It is somewaht difficult to precisely measure blood loss during and after delivery. Until large RCTs define its role in treating and preventing PPH, I'll continue using it alone or as an adjunct to other drugs.
