Informed consent IS a process, but the properly documented discussion had better have a statement in writing signed by the patient that she understood the risks of the chosen course of management and of other reasonable options. A physician-generated chart note or dictation statement although nice to include as an appendage to the process, is one-sided and self-serving unless it corroborates the substance of the written consent form.

The American Medical Association is no longer designating CME hours for AMA Category II CME credit.
