As I said previously, the only dose shown to be of any importance is the one just prior to surgery (I order it on call to the OR). I usually use ancef (cefazolin) 1g for cases involving entering both the vagina and the peritoneal cavity. For prolonged cases or those with significant blood loss there is some suggestion to using a second dose intra-op.
