I would be more concerned about a polyp or myoma than a malignancy. Therefore I wouold do an SIS.

An out of town relative of mine, not a patient, but a friend whom I supply freebie pills and advice (she does go to her doc for annuals) is a 45 YO on LoEstrin 1/20 for years. She called today to tell me that she is having a bit of BTB occasionally, not missing any pills, etc. Furthermore, her periods now start a couple of days earlier during the placebo week, and drag out longer.

Let's assume, for discussion, that her examination is normal, and her gyn history negative. She is a P2002, LLB 13 years ago. Does her bleeding history prompt any of us to investigate via endo biopsy, or perhaps SIS? My assumption is that even if she is perimenopausal, that the pill should control this, and that her bleeding patterns really should change with age while on the same OCP for years (except for those who become hypomenorrheic/amenorrheic over time, and this is not her pattern).
