If she continues to have cycles now while on the cyclic HRT, then I would leave it in for sure. If not, but there is any question leave it til age 60. I removed a Birnberg Bow from a 72 year old the other day. I inserted it in 1967 when I was a resident. I had not seen her in between.

50 YO inherited from another gyn who moved has been on HRT for 5 years. She initially was placed on cyclic HRT due to hot flashes, while she still was having spontaneous cycles, and has been a happy camper since.

When I saw her today for an annual exam, her Paraguard T was still in place from 1993. I suggested removal, and she stared real hard and asked if I could be *certain* that she wouldn't conceive if removed.

Gulp! My instinct said to take it out, but I chickened out and left it in, suggesting waiting until next year based on feeling and the ability to punt until next year.

Any thoughts, other than pointing out that I was a chicken?
